New Patients

First Visit

When you arrive for your first visit, we will need all insurance and health information forms completed before we can begin your dental treatment. We will ask you to fill out several forms that will get you acquainted with our office and help us get acquainted with you. Forms are available to download and print prior to your appointment or you can fill them out at our office prior to your appointment. Please plan to come in 15 minutes early if you need to complete these forms in office.

Also, please provide a list of the current medications you are taking, so we can avoid drug interactions. If you have a medical condition that requires you to take medicine prior to dental work, let us know.

Your initial exam will last approximately one hour. A comprehensive oral evaluation including x-rays will be provided to your existing dental health and proposed treatment.  A second visit will be scheduled with one of our hygienists for scaling and polishing.

Payment Options:

  • We accept Cash, Check, Visa, Mastercard, American Express and Discover.
  • Patients without insurance can enroll in our Dental Health Savings Plan. Click here to learn more!

Insurance and Co-Pays:

Our office is a contracted provider with Delta Dental. We accept most other insurance plans and we are happy to submit a claim on your behalf. We will always try to assist you in understanding your insurance benefits; however, the ultimate responsibility lies with you, the patient. If services are not covered due to reasons such as a waiting period, annual maximums, etc., then you are responsible for paying the charges.

Your estimated co-pay is due at the time of service. Should you have trouble taking care of your co-pay, arrangements must be made with one of our front office professionals prior to your appointment.

Most importantly, please keep us informed of any insurance changes such as ID numbers, new insurance company, or a change of employment.
